DSW SUPERVISOR JOB DESCRIPTION

Hours: Monday-Friday 8:00 – 4:30 pm.
Every other Friday off at noon, unless needed for job
commitments.
EDUCATION:
High school diploma required, some college preferred.
Any healthcare education a plus.
EXPERIENCE:
MUST HAVE WORKED AS A SUPERVISOR FOR A MINIMUM OF 1 YEAR.
PREFERABLY IN THE
HEALTHCARE INDUSTRY.
OTHER SUPERVISORY EXPERIENCE A PLUS.

· Must have reliable transportation, valid driver’s license and safe driving record
Agree to preserve the confidentiality of all clients, their families, staff management & company information
Experienced at using a laptop, emails, scanning, copier & fax
Must adapt quickly to new computer programs.
JOB FUNCTIONS/RESPONSIBILITIES:
· Conduct in-person visits with each DSW, required to evaluate performance, client status changes, and emergency drills conducted to ensure services provided per ISP requirements.
· Supervisor shall visit the facility/home of client on a quarterly basis, checking if service plan is still needed, if it is adequate or needs any revisions.
Ensure all documents have correct signatures and dates.
Set up state audits.

· Supervisor must verify DSW visits daily to ensure services are provided in accordance with the ISP.
· If DSW positions cannot be staffed, the DSW Supervisor must report to work.
· Supervisor is responsible for answering after hour & staffing calls on rotation.
· Setting up client binders, check yearly.
Attend quarterly meetings.
· Supervisor must complete accident/incident reports within 24 hours.
Report all accidents/incidents & complaints to Manager ASAP.
· Able to read & understand client’s POC, type the client’s ISP & Universal POC.
· Create schedules for DSWs, initially, annually and as needed.
· All client & employee documents must be scanned into data base and filed immediately.
Conduct weekly PA checks to ensure all clients have a valid PA.
· Handle filing, generating reports, set up meetings, answer phones, ordering supplies.
· Immediately report any indication of a potentially significant issue developing.
· Retain proper documentation regarding all client and staff communication.
· Any additional job related duties as assigned by the Manager or Executive Director.
