Home Health Clinical Manager

Responsibilities: Clinical Manager is an individual qualified to provide oversight of patient care services and personnel.
Oversight includes making patient and personnel assignments, coordinating patient care, coordinating referrals, ensuring that patient needs are continually assessed and ensuring the development, implementation, and update of patients' plans of care.
Auditing of clinical charts.
Preparing nurse's schedules.
Leading PROP meetings.
Ordering medical supplies.
A clinical manager must be available during all operating hours.
Requirements: - Must be an RN, Physical Therapist, Speech Pathologist, Occupational Therapist, Social Worker or Licensed Physician.
- Must have home health/Oasis experience.
- Strong interpersonal skills and the ability to effectively communicate with individuals of all ages and backgrounds.
- Ability to work independently as well as collaboratively within a team setting.
- Excellent organizational skills and attention to detail.
