STAT Home Health is seeking a Full-Time Palliative Care Licensed Practical Nurse (LPN) to join our patient care team in Lafayette, LA.
As a palliative care nurse for STAT Home Health, you would also become a dual employee for St.
Joseph Hospice and make Hospice home visits as needed.

Essential duties include: Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN, and AIM Coordinator Communicates with the attending physicians, medical directors, other home health and hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family Document all services provided and submit timely on appropriate device/forms Demonstrates knowledge of Medicare/Medicaid regulations for home health and hospice Report changes in patient condition and needs to RN Maintain excellent time management skills Participate in team meetings/case conferences as scheduled Demonstrate compassion and sincere interpersonal skills Qualifications: Minimum of 2 years as a Licensed Practical Nurse (LPN) Current Louisiana license Must be able to work a flexible schedule Have dependable transportation, driver’s license, and car insurance Home Health, Hospice, Palliative care experience preferred Full-Time Benefits: Competitive Pay Increased Earning Potential Health, Dental, Vision & Life Insurance Short-term & Long-term Disability 401(k) Paid Time Off Cell Phone Allowance Mileage reimbursement (reflective of fluctuating fuel prices) Exceptional People providing Exceptional Care.
